Abstract

Abstract. Introduction. In the pathogenesis of many diseases of pregnant women, a significant role is given to the systemic inflammatory response and oxidative stress, due to the imbalance between prooxidants and the antioxidant defense system. Currently, acute pyelonephritis during pregnancy is one of the most common diseases of the urinary system. In the literature, there are practically no data on the study of the state of the pro- and antioxidant system in acute pyelonephritis in dynamics in each trimester of pregnancy. Objective of the study: study of the state of the pro- and antioxidant system at the systemic and local level in acute serous pyelonephritis in different trimesters of gestation. Material and methods. The study included 130 women (mean age 25.1±4.3 years), divided into groups: comparison (20 healthy women), (20 patients with a verified diagnosis of acute serous pyelonephritis), (15 women in the 1st, 2nd and 3rd trimesters of gestation); main groups (15 women in each) with acute pyelonephritis in the 1st, 2nd and 3rd trimesters of pregnancy. The oxidant-antioxidant system parameters in plasma and erythrocytes of peripheral blood and urine were studied. Results. In acute pyelonephritis in all trimesters of gestation, to a greater extent in the second and third, the development of systemic inflammation, oxidative stress and endothelial dysfunction has been established, since at the systemic (peripheral blood plasma) and local (erythrocytes and urine) level, the content of malondialdehyde, acyl hydroperoxides, neopterin, C-reactive protein, stable metabolites of nitric oxide is increased, antioxidant defense factors and endothelin-1 levels are reduced.
