Цитохром Р450 и вспомогательные репродуктивные технологии: от молекулярных основ метаболизма до персонализации протоколов стимуляции суперовуляции

Введение. Ферменты суперсемейства цитохрома P450 (CYP) играют важную роль в реализации стероидогенеза, регуляции естественного менструального цикла и контролируемой овариальной стимуляции, при этом их активность находится под контролем гипоталамо-гипофизарной системы. В связи с этим анализ полиморфных вариантов генов CYP рассматривается как перспективный подход к прогнозированию исходов программ вспомогательных репродуктивных технологий (ВРТ). Кроме того, актуальность исследования обусловлена значительной индивидуальной вариабельностью ответа яичников на контролируемую стимуляцию овуляции в протоколах экстракорпорального оплодотворения, что диктует необходимость углубленного изучения ее молекулярно-генетических детерминант. Цель обзора — провести сравнительный анализ клинической значимости полиморфных вариантов генов CYP (CYP2A6, CYP2C9, CYP2C19, CYP2D6, CYP3A4, CYP19A1) для персонализации этапов протокола экстракорпорального оплодотворения: от выбора и дозирования индукторов овуляции до поддержки лютеиновой фазы.
Методика. Проведен структурированный поиск литературы в базах данных PubMed/MEDLINE, Scopus и РИНЦ за период 2014–2025 гг. Из 783 первоначально выявленных публикаций после скрининга и оценки методологического качества в итоговый анализ вошел 71 источник.
Результаты. Генетические вариации цитохрома Р450 модулируют клиренс и эффективность гормональных препаратов, что напрямую сказывается на таких параметрах, как количество и качество полученных ооцитов и эмбрионов, частота наступления беременности и риск развития синдрома гиперстимуляции яичников. Изученные полиморфизмы располагаются согласно иерархии своей клинической значимости: наибольшей доказательной базой и готовностью к внедрению обладают тесты на CYP19A1 (предиктор овариального ответа и риска синдрома гиперстимуляции яичников) и CYP2D6 (маркер эффективности кломифена цитрата).
Заключение. Применение приоритизированной модели интеграции фармакогенетического тестирования CYP в рутинную клиническую практику позволяет оптимизировать режимы дозирования гормональных препаратов, выбирать альтернативные лекарственные средства, своевременно корректировать тактику ведения пациенток с высоким риском неблагоприятных исходов, повышать эффективность программ ВРТ и снижать частоту ятрогенных осложнений.
